I saw this one at the 2011 Fall Philly guitar show. Dealer Grinning Elk Music graciously allowed me to snap a few pics, as shown here. Very clean example. As far as I can see the washer on the input jack is precautionary as I don't see any signs of cracking around the input jack area.
