Here's another rare fretless bass with black appointments, up for auction on Ebay in September 2015. Nice example, with only minor flaws such as a chip in the pickguard. No mention of an OEM case...

"This auction is for a very cool vintage Bass. What separates this one from most is that 1) it is the very, very rare fretless version and 2) this was purchased new by professional Jazz Bassist Bob Davis and kept in a collection, not being used on the road which is why there is really no visible wear. We purchased it from his Nephew who was selling off the collection. It was relayed that this was a special order and is an absolutely original piece and period correct. Serial number is D2554A. This is being sold only to help fund one of our Daughters upcoming wedding. We will be shipping it through a local UPS Store who will pack it correctly and ship it professionally. Please look at all the pictures and use the zoom option to see how fine this one is."
