Here's an interesting package that hit the market in May 2015. While the bass itself looks to be in good shape and fairly original, the extra bass pickup adds a lot of value to the package. We're never seen an extra bass pickup, let alone one in such good cosmetic shape. Does it work? Too bad the seller hasn't taken it to a reputable shop to have it tested, because if it does work it's going to be worth a lot.
The other item to note is that this bass appears to have been retrofitted with a tone switch, possibly in keeping with Ampeg's updated spec for guitars/basses produced with serial numbers above 2000. Certainly, a bass produced with serial number D1058A would not have originally come with a tone switch. You can also tell this is a retrofit by the position of the switch, which is a little higher (i.e., closer to the peghead) than basses with factory switches.

This bass was purchased in the late 60's in San Francisco, CA by a local musician and played throughout the 60's and 70's.
