D1241A
This parts project came up for auction on eBay and after going through several listings eventually sold for $510 in July 2012. Shame about the extra holes in the headstock, but a competent luthier should be able to sort it out and turn it back into a playing instrument. In any event, there's at least $500 worth of parts here so probably can't go too far wrong on the purchase.
"Original Dan Armstrong bass # D1241A, replacement pick guard and headstock cover, replacement tuners, at some point it had 2 more holes drilled in headstock but they have been filled back in, great project guitar, comes with original pickup, vintage vox case."

Looks like someone was able to put it all together and come up with a playable instrument. Was listed again on ebay and sold for $610 on December 29, 2013. Seller said: "Dan Armstrong Lucite Plexi Bass 1969. Serial number: D1241A. Two extra holes were drilled in headstock but later filled. New faceplate included to change the old one. Pickguard is not original. Hardcase included. Great bass, plays like a charm."
